Correction of Birth Record
 
Detailed Information and Requirements

If a birth record has not been recorded or has been lost or destroyed or has not been properly and accurately recorded, the Probate Court, upon application, may cause the record of birth to be recorded or corrected. The application needs to be supported by affidavit and other proff as may be required.

You must be present in front of the clerk of this Court with a certified copy of your birth certificate.  At that time, the clerk will complete the necessary form, prescribed by the Ohio Department of Health, for you. The form must then be signed by you.  The clerk will then sign and seal the form and mail the form to the Ohio Department of Vital Statistics for correction.  Upon completion the clerk will also provide you with a certified copy of the application for correction of birth.

The fee for the correction is $35.00 and MUST be paid at the time of application. Payment can be made in cash or by credit card. NO PERSONAL CHECKS WILL BE ACCEPTED.

 
All forms must be typewritten, filled out completely and signed ONLY in blue ink.
Instructions for completion of Corrections of Birth Record Form
                 
                 
  To apply for a Birth Record correction through this Probate Court, you must have been born in Ohio and either have:    
  1.   been born in Ross County,        
  2.   your mother was a resident of Ross County at the time of your birth or,
  3.  you now reside in Ross County,        
  You must present the Court with two valid documents to prove the information that you wish to correct. Below is an example of evidence that may be used in this Court:
 
  • Certificate of Baptism
       
 
  • Federal Census Records
   
 
  • School records or transcripts, with an original signature from the records secretary
 
  • Honorable Discharge from U.S. Armed Forces (DD 214)
 
  • Certified copy of application for marriage license, showing vital information
 
  • Life insurance policies (still in effect), at least four years old, showing date and place of birth
 
  • Letter from hospital or doctor (indicating vital information) on letterhead
 
  • Notarized affidavit of a witness to the birth
 
  • Certified copy of one child's birth certificate
             
  To correct the sex on an adult's birth certificate, a doctor's letter (on letterhead) stating that no surgery for sexual reassignment has taken place, is mandatory.  
                     
  Birth Correction          
  The fee for registration is $37.00 and MUST be paid at the time of application.  Fee can be paid in cash or by credit card. No Personal checks will be accepted.    
Detailed information and requirements regarding changing race status from caucasian to American Indian.      
   
  • The birth must have occurred in Ross County, Ohio or your mother resided in Ross County at the time of your birth.
 
  • When you want to correct your race from caucasian to American Indian, you MUST provide the original card from the Indian Tribe which they are a member of, stating that you are a member.
 
  • You also MUST provide the paperwork from the Tribe stating their heritage.
 
  • You must present yourself before the clerk of this Court and bring with you a certified copy of your birth certificate and the aforementioned Tribe information.
 
  • The clerk will then complete the top section of the form prescribed by the State.
 
  • The form must then be signed by you. The clerk will t hen sign and seal the form and mail the form, along with a copy of the Tribe card, to the Ohio Department of Vital Statistics for correction.
 
  • Upon completion, the clerk will also provide you with a certified copy of the application for correction of birth.
                       

                         
Application for Registration of Birth
Instructions for Registration of Birth

To apply for a Delayed Birth Registration through this Court, you must have been born in Ross County, State of Ohio or reside in Ross County and was born in the State of Ohio. You will need to provide the Court with a "No-Record" letter from the Ohio Department of Health, Vital Statistics. (NOTE: For births that occured prior to December 30, 1908, please contact the Probate Court in the county where the birth occured for the search.) Once you have this "No-Record" letter, you must present the Court with at least three (3) valid documents to prove the exact date and place of your birth and your parents' names. Below is an example of evidence that may be used in this Court:

   
  • Certificate of Baptism
